This form allows us to:

  • Reschedule your delivery date and time
  • Confirm updated delivery instructions or contact info
  • Ensure you’re notified of any additional delivery fees (additional fees apply) 

When to Use This Form

  • You weren’t available to receive the delivery
  • The carrier attempted delivery but access was restricted
  • Your freight shipment was turned away or redirected
  • You’d like to arrange delivery at a different time or address
