This simple form confirms that you’ve carefully inspected your shipment for any visible damage, missing items, or packaging issues at the time of arrival.

Filling out this form helps:

  • Verify your order arrived safely and in full
  • Document any damage or discrepancies immediately
  • Support any claims made with the shipping carrier if needed

Why it matters:

Shipping damage is rare, but when it happens, proper documentation is key. This form ensures you’re protected and helps us resolve issues quickly and fairly.

Please inspect your shipment before signing and submitting the form.

If anything is damaged or missing:

  • Note it clearly on the delivery receipt or Bill of Lading (BOL)
  • Take clear, high-quality photos of all affected areas
  • Submit this form with all necessary documentation to support a claim

Do I Have to Fill Out the Shipment Inspection Acknowledgment Form?

This form is only required if:

  • Your order was delivered via freight shipping (large or commercial equipment)
  • There is visible damage to the packaging or product at the time of delivery
  • You are receiving the shipment on behalf of a business or facility

For smaller items shipped via standard carriers (UPS, FedEx, USPS), this form is not required unless there is visible damage upon arrival.
